CPR, Physical Exam, Criminal Background Check
Hello, I am Amy Lee. I will be your primary faculty member for N302 Health and Physical Assessment. I look forward to working with you during your sophomore year, and again as clinical and guest faculty throughout the BSN degree requirements. I will be the faculty member who will coordinate your personal health physical examination forms, Cardopulmonary Resucitation (CPR) certification and criminal background checks. I would like to discuss with you those requirements.
PHYSICAL EXAM
Current requirements for student infectious disease testing/immunization and CPR are documented in the School of Nursing Physical Examination Form. This form was mailed to you with your letter of acceptance to the School of Nursing. You may also find a copy of the New Student History and Physical Examination Form in the forms section of the SON student handbook, which is found on our website, www.odu.edu/nursson. This physical must be completed during the month of August. Students will then submit this physical form to nursing faculty during new nursing student orientation in August. DO NOT mail this form to the School of Nursing! Please do not plan on submitting original copies of your documents. Submit only copies.
Immunizations and Proof of Serum Immunity
Tuberculosis Skin Testing (PPD)
Students are required to have PPD skin testing during the month of August. Students must also complete the Tuberculosis(TB) surveillance questionnaire found within the Student physical examination form. If students have had a past positive positive PPD result, or received the BCG immunization during childhood. Documentation of:
- TB prophylaxis received, and
- negative Chest X-ray within the last year must be submitted with health physical form
Hepatitis B (HBV) Vaccination
Students are required to submit proof of immunization of the Hepatitis B vaccination series, and proof of serum immunity (positive titer) to the disease. Students who have not received the 3 vaccination series in the past, are only required to submit proof of vaccination #1. Students must complete the series by March or will not be able to participate in the N303 clinical rotation. If students have completed the series in the past and do not have serum immunity to the disease, they must consult their health care provider and begin the 3 vaccination series again, or receive a booster vaccination.
Tetanus/Diptheria/Pertussis Vaccination
Students must submit proof of Tdap within the last eight years.
Rubeola (Measles), Mumps, Rubella, and Varicella
Students must submit proof of serum immunity (positive serum titer) to these diseases. Proof of having the disease will not suffice. If students do not have serum immunity (negative titer) to any of the above diseases, documentation of the MMR and Varicella vaccination x2 must be submitted.
CPR CERTIFICATION
All prelicensure students must successfully complete CPR / Basic Life Support Certification (1 man, AED, 2 man, adult, child) in the month of August prior to beginning the nursing major. Students are required to maintain the certification while a student in the program. Documentation of continued certification must be submitted to the School of Nursing by the first day of fall semester classes each year.
CRIMINAL BACKGROUND/SEX OFFENDER STATUS CHECK
Students receive information about the process for the Criminal Background / Sex Offender Status check with their School of Nursing letter of acceptance. This form may also be obtained from Ann McNeal (Administrative Assistant) in the Health Science Building, Room 2134. Instructions for submission are included. Students must have the results come to them. DO NOT have the results mailed to the School of Nursing. Print several copies of the results and put a printed copy in each of several envelopes (6 suggested). Seal each envelope, sign across the back flap and store in a secure place. Have an envelope ready to submit to any health care agency requiring it.
